Use this form to notify CDF of the death of an account holder, accompanied by a certified copy of the deceased's Death Certificate.

Death Certificate Requirements

Please review the death certificate requirements below:
- A certified copy of a registered Death Certificate is mandatory
- CDF cannot accept a 'cause of death' certificate as a substitute

Eligibility

This form can be completed by any party notifying CDF of the death of an account holder, including:
- Next of Kin
- An executor or administrator of an estate
